Helpful Information


The ROOTS Clinic is in network with Premera Blue Cross and Regence Blue Shield. Premera includes self-funded plans such as the Amazon and Microsoft Premera plans. Lifewise plans are also included in Premera. If you have coverage with any of these carriers, we may be able to bill your insurance directly for ABA services rendered.


To find out more, you may submit paperwork for a benefit verification:

  • Demographic form (Link to PDF)
  • Copies (photo or scan to email preferred) of the front and back of insurance cards for all active plans
  • Copy of diagnostic evaluation report with autism diagnosis

It’s very important that information on all plans is submitted because if there are multiple plans, the benefits will have to be coordinated to identify which plans are primary, secondary, and tertiary before claims can be processed.

Some things to note:

  • ABA coverage does not apply unless a diagnosis of autism or a related disorder (such as PDD-NOS) exists and is current
  • ABA coverage does not exempt you from the base tuition costs which are separate and charged via private pay, and not eligible for insurance coverage
  • Many plans have an exclusion against dual coverage of two agencies (i.e. if one agency is serving in the home and one agency is serving in the school location)
  • If your coverage changes mid-schoolyear (i.e. if your employer changes or your plan changes in January to a different carrier that does not cover our services) you will be responsible for the full private pay cost of services effective immediately upon the change

If you are insured with a company other than Premera or Regence, we are happy to complete a benefit check and work with you and the insurance company to determine if we can provide services as an out-of network provider, or obtain a single case agreement.
